  • Hand Sanitizer Parent Opt. Out
  • Hand Sanitizer Staff Assist
  • COVID19 Positive Parent-Reported
  • COVID19 Medical Risk Factor
  • COVID19 Presumed Immunity per HCP
  • Vaccinated for COVID19
  • Exempt from COVID19 Vaccination
